How to be referred

If you have a health condition and want to get more active, please request an Active 4 Health exercise referral from your GP, Physiotherapist or other health professional. 

You qualify for an exercise referral if you have one or more of the following health conditions:

  • Arthritis 
  • Asthma or COPD (Chronic Obstructive Pulmonary Disorder) 
  • Back pain 
  • Cancer (pre, during or post treatment) 
  • Chronic fatigue and ME 
  • Coronary artery disease or peripheral vascular disease 
  • Dementia or Alzheimer’s 
  • Fibromyalgia and Polymyalgia 
  • General mobility issues/physical disability 
  • High blood pressure 
  • High Cholesterol 
  • Long Covid 
  • Mental ill health, i.e. stress, anxiety and depression 
  • Multiple Sclerosis 
  • Osteoporosis 
  • Overweight or obesity 
  • Parkinson’s disease 
  • Post-surgery 
  • Previous heart surgery or heart attack (must have completed phase 3 cardiac rehab) 
  • Risk of falls/balance issues 
  • Stroke / brain injury 

You also qualify if you have 3 or more of the following lifestyle factors: 

  • Smoker 
  • Prediabetes 
  • Family history of heart disease 
  • Overweight with a BMI below 30 
  • High Cholesterol 
  • Sedentary lifestyle
  • High blood pressure (140/90) 

Once we receive your referral from your health professional, you will be contacted by our Lifestyle Coordinator as soon as possible to complete an initial lifestyle screening over the phone.  

This will enable us to help you get started on your active journey. Sometimes there is a delay in receiving referrals from health professionals, so we suggest you wait 10 days before contacting us.

After your lifestyle screening, we will refer you to the agreed pathway based on your location, interest, and health needs.
